Reason for Alert: Intravenous (IV) Fluid Shortage

Potential risks of the stated issue: This practice alert is to call attention to the probable risk for a
shortage of IV fluids produced by Baxter International, Inc., in North Carolina, due to damage
incurred by Hurricane Helene. Baxter International, Inc. is one of the leading suppliers of IV fluids in
the United States (American Hospital Association [AHA], 2024). According to a press release from
Baxter International, Inc. they are working with several global plants to try to remedy this situation
(Baxter, 2024).

Recommendations for practice: It is imperative for the nursing profession to keep abreast of this
potential shortage and identify strategies to help conserve IV fluid.

Strategies recommended are as follows:

  • Assess individual needs to continue intravenous fluid replacement prior to administration, at every shift change, and prior to starting a new bag.
  • Discontinue IV fluids as soon as clinically indicated.
  • Consider saline locks for eligible patients (flushing per policy).
  • When possible, use oral electrolyte and hydration.
  • Consider administering antibiotics and other IV medications via IV push versus drip when possible.
  • Avoid opening and preparing bags in anticipation of use.
  • Use smaller volume bags for low infusion rates.
  • Take stock of inventory and develop alternative treatment plans when possible.
  • Develop temporary policies for substitution of IV fluid based on hospital supply (e.g.,allow substitution of normal saline IV fluid for Lactated Ringers depending on hospital supply).
  • Stay up to date with current supply recommendations.
